About the procedure
Therapeutic bloodletting — sometimes called therapeutic phlebotomy — is the controlled, clinical removal of a measured volume of blood. It is a procedure with a well-established history in medicine, used today primarily in the management of elevated red blood cell mass or iron overload.
The method is comparable to a standard blood donation. A trained phlebotomist draws blood through a sterile venepuncture into a clinical waste bag for safe disposal. The full appointment typically takes 30–45 minutes from start to finish.
Unlike donation, this is a medically indicated procedure. It is not appropriate for everyone, and eligibility is always determined before any appointment is arranged — based on your recent blood results, health history, and individual circumstances.

All blood and sharps waste is collected and disposed of via a registered clinical waste contractor. Nothing is left at your address.

This procedure may be appropriate for individuals in specific clinical circumstances. Assessment is required before any appointment is arranged.
Testosterone replacement therapy is a common cause of elevated haematocrit. Individuals whose prescriber has recommended haematocrit reduction may be suitable candidates.
Individuals whose FBC shows haematocrit above clinically accepted ranges, directed to address this by a registered practitioner, will be considered following eligibility review.
Elevated serum ferritin indicating increased iron stores may be addressed through therapeutic phlebotomy. Suitability depends on the underlying cause and full blood picture.
If your GP, TRT clinic or specialist has directed you to undergo therapeutic phlebotomy and access has been difficult, we may be able to assist following clinical review.
A note on reported benefits: Some individuals report improvements in comfort, wellbeing and pressure-related symptoms following the procedure. This varies by individual and is not guaranteed. This service does not replace the management plan of your prescribing clinician.

Preparation & aftercare
Everything you need to know before, during and after your procedure.
Before your appointment
Drink plenty of water, eat a light meal and wear comfortable clothing with easy access to your inner arm. Avoid alcohol for 24 hours beforehand.
During the procedure
Your phlebotomist confirms your identity and consent on arrival. The blood draw takes 15–20 minutes. You remain seated or lying down throughout.
Immediately after
Rest for 10–15 minutes before standing. Your phlebotomist monitors you before leaving. Have a light snack and drink ready. Avoid strenuous activity for the rest of the day.
Recovery at home
Most individuals resume light activity within a few hours. Some temporary fatigue is normal. Stay hydrated and avoid heavy lifting or driving for the rest of the day.
Follow-up testing
We recommend retesting your FBC 4–6 weeks after the procedure. We can assist with follow-up blood testing through our laboratory service if needed.
When to seek help
Contact a healthcare professional if you experience prolonged dizziness, unresolved bleeding or unusual chest pain. If in doubt, contact 111.
Transparency & safety
As with any procedure involving venepuncture, there is a small number of associated risks. We believe in being fully transparent about what they are.
The most common is bruising at the venepuncture site, which typically resolves within a few days. Transient dizziness on standing is also possible, as is a vasovagal response — a fainting reaction — which is rare but more likely in individuals with a sensitivity to needles.
Temporary fatigue for several hours following the procedure is normal. Infection risk is minimised through sterile, single-use equipment and aseptic technique throughout. In rare cases, venous access may be technically challenging and the procedure may need to be deferred.

Common questions
A recent Full Blood Count (FBC) including haematocrit and haemoglobin values, ideally within the previous 4–6 weeks. If you are on TRT, ferritin levels are also helpful. Results from an NHS or registered UK laboratory are accepted and must be submitted as part of your application.
Volume is determined during clinical review based on your results, weight and health status. Typical volumes are comparable to a blood donation — approximately 450ml — though this may be reduced depending on your individual picture. The volume is confirmed with you before the procedure begins.
Many of our applicants are currently receiving testosterone replacement therapy. Elevated haematocrit is a well-documented response to TRT. Your application will be reviewed in the context of your blood results and medical history. TRT use alone does not guarantee approval, but it is one of the most common reasons individuals apply.
A formal referral is not a strict requirement, but written direction from your GP, TRT clinic or specialist will strengthen your application. We recommend letting your prescribing practitioner know you are seeking this procedure regardless.
